This service administers Admission Discharge Assessment Tools including the Basis 32 and Drug Use Inventory for patients seeking access and referral to substance use services.
Additional psychosocial information is gathered through the Central Intake process. The service utilizes the Stages of Change Model to assist in identifying the level of care the patient requires.
This service is 15-days and consists of structured, scheduled group therapy with a focus on substance abuse treatment activities. This service is offered five days per week to clients residing at home or at another supportive setting within the community.
The service utilizes a client-centered, bio-psychosocial approach, utilizing various tools and strategies such as CBT, harm reduction, stages of change and relapse prevention.
This outpatient relapse prevention service is offered to clients who have completed a 21-day residential treatment program and want the opportunity to enhance and maintain learned coping strategies as they reintegrate into the community. The six session service consists of support which is client-centered, uses a bio-psychological approach, CBT, and harm reduction strategies.
